All payments must be received before class.

First Time-Trial Classes are free.

Classes do not carry over each month. Students must re-register at the start of each new month.

Liability Waiver and Consent Form

New students are permitted a total of ONE trial class.

Trial classes are only offered on regularly scheduled class dates (Fridays and Saturdays).

MAGIC Dance Arts has the right to deny your request for a trial class if the class session has already reached maximum capacity. If this is the case, we will contact you with further details.

CONSENT FORM & LIABILITY WAIVER

Parent, Legal Guardian, and Participant understand & agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks & responsibility for any such injury or accident, which might occur to me/my child during any of the classes, rehearsal, performances, or activities provided by MAGIC Dance Arts. I also exempt, release, & indemnify MAGIC Dance Arts, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, &/or students from any & all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my child, or property which may arise out of or in connection with participation in any classes or activities conducted by MAGIC Dance Arts. I further hereby voluntarily agree to waive my rights & that of my heirs & assigns to hold MAGIC Dance Arts, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my/my child’s physical limitations & agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian & have the right to waive these rights. I hereby grant permission to MAGIC Dance Arts to use photographs & video footage of students for publicity purposes. I have read, understood, & agree to be bound by the above statement and conditions.
